Introduction
My participation in skating activities at the Ice Peaks Holon rink, operated by Psagot HaKerech Ltd., is conditional upon my review of the skating approval conditions and giving my consent (or as a parent/guardian for my minor child). Filling out my details means I have read and agree to all conditions.
I am aware that I am not legally obligated to provide these details, but without them, I cannot participate in ice skating activities.
Use of the Skating Rink
I am aware that for safety reasons, entry and use are conditional on:
- Watching and listening to safety instructions displayed and announced at the rink.
- Agreeing to the site regulations and this approval.
- Compliance with published rules of conduct and Ice Peaks staff instructions ("Psagot Rules").
I undertake to refresh my memory regarding these rules on every visit. I understand that Psagot may deny my entry or remove me if I fail to comply.
Health & Safety
- I am healthy and do not suffer from a medical condition that may worsen due to the activity.
- I am not pregnant.
- Skating under the influence of drugs or alcohol is strictly prohibited.
- Food, drinks, and animals are not allowed on the ice surface.
Liability Release
I understand that skating involves inherent risks, including falls and injuries. I accept these risks and choose to participate of my own free will. I hereby release Psagot and its representatives from any liability for injury, loss, or damage caused to me during the activity.
Photography
I am aware that the rink is monitored by cameras. I consent to Ice Peaks using photos/videos in which I or my children appear for any purpose.
General
- Adults (18+) sign for themselves. Minors require a parent/guardian signature.
- An electronic signature is valid indefinitely (subject to company discretion).
- I agree that my details will be stored and used for marketing purposes (according to the Privacy Policy).
Skating Aid ("Bobby/Seal")
Before using the skating aid, I declare that:
- Sitting on the aid or carrying people is prohibited.
- Use is allowed only for children under 10 with close adult supervision.
- Availability depends on rink load and management discretion.
- If demand is high, use is limited to 15 minutes.
- Responsibility for any damage due to improper use lies solely with the user.
I confirm the above by signing this approval.
